Academic Dishonesty

Faculty, please report all cases of Academic Dishonesty through the form below.  This form should be used solely for incidents of academic dishonesty.

Once you submit the form, a check for prior history will be conducted and you will be contacted with the appropriate next steps. The Informal Resolution Form will be sent to you to resolve the case yourself or request additional documentation of the violation for the case to be heard and resolved within the Provost’s Office and/or the Office of Student Conduct.
