FORMS FOR PI USE | |
Institutional Commitment Form | Complete this form with assistance from your grant coordinator if you anticipate any project-specific institutional commitments. The grant coordinator will circulate for approval signatures. This form MUST be signed PRIOR to the full Internal Approval process. |
Consultant Information | Complete for consultants with limited scope (e.g. advisory board, expert panel) and send to grant coordinator. See Subrecipients/Vendors/Consultants for additional information. |
Consultant Letter of Commitment – Individual | Secure completed/signed letter from a consultant with a more expanded role (but not to the level of a subrecipient) and send to grant coordinator. A project evaluator may fall into this category. See Subrecipients/Vendors/Consultants for additional information. |
Subrecipient Information | Provide basic information to grant coordinator for a subrecipient that requires an Institutional Letter of Commitment. See Subrecipients/Vendors/Consultants for additional information. |
Limited Submission Coversheet | Use for internal limited submission competition AFTER Research Services has determined that an internal competition is necessary; see Limited Submission section. |

Budget Template | This budget template will allow you to calculate a draft budget. You may or may not choose to use this template for the internal approval process and submission, depending upon sponsor format. |
Budget Justification | The budget justification template contains standard language that can be used for generation of a budget justification or narrative. |
NSF Biographical Sketch (used to assess how well qualified the individual, team, or organization is to conduct the proposed activities) | The biographical sketch must be provided separately for each individual designated as a senior/key person through use of SciENcv (Science Experts Network Curriculum Vitae). Senior/key personnel must prepare, save, certify, and submit these documents as part of their proposal via Research.gov or Grants.gov. SciENcv FAQs |
NSF Current and Pending (other) Support (used to assess the capacity or any conflicts of commitment that may impact the ability of the individual to carry out the research effort as proposed and helps assess any potential scientific and budgetary overlap/duplication with the project being proposed.) | Current and pending (other) support information must be provided separately for each individual designated as a senior/key person through use of SciENcv (Science Experts Network Curriculum Vitae). NSF Collaborators and Other Affiliations | Collaborators and Other Affiliations document contains a table of an individual’s collaborators, such as their advisors, co-authors and students. NSF requires the use of the Collaborators and Other Affiliations Excel Template for identifying this information. |
NSF Synergistic Activities | Synergistic Activities is a required one-page document for each individual identified as senior/key personnel that includes a list of up to five distinct examples that demonstrate the broader impact of an individual’s professional and scholarly activities, focusing on the creation, integration and transfer of knowledge. |
NSF Letter of Collaboration | If required or allowed, an NSF Letter of Collaboration has a set format. It is different than a Letter of Support. |

FORMS FOR GRANT COORDINATOR USE | |
Consultant Letter of Commitment – Institutional | Complete for a consultant with a more expanded role (but not to the level of a subrecipient) based on information provided by the PI via the Subrecipient Information form. |
Subrecipient/Contractor Determination | If the recipient is not a consultant, the GC will document the final subrecipient/contractor determination using this form. |
Subrecipient Risk Analysis | Used to evaluate and assign a level of risk for each subrecipient. |
FDP Subrecipient LOI | For institutions participating in the FDP Clearinghouse, this letter of intent is used for subrecipient commitments on sponsored programs. |
Non-FDP Subrecipient Commitment Form | For institutions not participating in the FDP clearinghouse and serving as a subrecipient on sponsored programs. |

Preliminary Request for Off-Campus F&A Rate | Share with the PI for completion upon request from PI for an off-campus F&A rate that may be justifiable. This form will require internal provisional approval prior to using an alternative rate in draft budget preparations. |
